R wave
Finally, any downward deflection after the R wave is called the S wave. If the next deflection is upward, or positive, then it's called the R wave. If, though, the first wave after the p-wave is upwards, instead, or positive, you basically skip the Q and just called it an R wave. Lead V6 sees the opposite, producing a wide, characteristic " bunny ears" QRS complex with two R waves. As in this example in a lead in which the QRS complex ends with an R wave. Lead V1 gives a characteristic M shape with a terminal R wave, while V6 sees a broader S wave. Lack of depolarizing flow through the dead segment results in a reduction of R wave activity. In addition, there is a gradual increase in amplitude of R wave from V1 to V5. In this case it may be helpful to look at R wave progression. This is known as R wave progression. So, when an ectopic focus originates in the left ventricle, and moves towards the right ventricle, the V1 lead shows a large positive complex, with a dominating R wave. R wave is produced by depolarization of the main mass of the ventricles. Non-progression or poor progression of R wave - R wave stays low and S wave remains deep throughout all chest leads. For rhythm: measure the intervals between the R waves. This is the most common finding, or as illustrated here, inversion of T waves in leads with dominant R waves loss of concordance. Reverse progression of R wave - tall R wave in V1, tallest in V1 or V2 - is usually seen in right ventricular hypertrophy. Such early Q waves may disappear from the patient's ECG, a normal R wave progression may be restored if vessel patency is reestablished within the appropriate time frame. In a given ECG lead, T waves are generally concordant with the QRS complex so that in a lead with a dominant R wave such as v6 the T waves are normally upright. It is important to be aware that in a proportion of cases Q waves and loss of R wave activity may develop very early in the course of STEMI.
QRS complex![]() ![]() ![]() The QRS complex is a name for the combination of three of the graphical deflections seen on a typical electrocardiogram (EKG or ECG). It is usually the central and most visually obvious part of the tracing. It corresponds to the depolarization of the right and left ventricles of the human heart. In adults, it normally lasts 0.06–0.10 s; in children and during physical activity, it may be shorter. The Q, R, and S waves occur in rapid succession, do not all appear in all leads, and reflect a single event, and thus are usually considered together. A Q wave is any downward deflection after the P wave. An R wave follows as an upward deflection, and the S wave is any downward deflection after the R wave. The T wave follows the S wave, and in some cases an additional U wave follows the T wave. |
