医疗上的禁忌（Contraindication）也称为禁忌症，是指不对病患进行特定治疗方式的原因，其原因多半是因为病患的症状或因素，若进行治疗方式会对病患造成损伤，因此不进行此治疗方式，禁忌症和适应症恰好相反，后者是使用某一治疗方式的原因。 有些禁忌症是绝对的，表示不论如何，在此情形下都不允许使用此治疗方式。例如青少年及儿童的病毒感染不能使用阿斯匹灵，因为会有雷氏症候群的风险 ，若病患对某一食物有全身型过敏性反应，严禁食用会造成过敏的食物，若病患有血色沉着病，严禁补充铁剂。 有些禁忌症是相对的，表示若使用此治疗方式，病患虽然有较高的风险，但风险可因为其他因素抵消或是用一些方式缓和。例如孕妇一般是避免照X光，但若为了诊断（并治疗）一些严重疾病（结核）而照X光，其助益可以对抵消X光的风险。相对性的禁忌症一般会用「用药须知」（cautions）来表示，就像英国国家处方集的作法。
